Position Title: Program Coordinator
Division: Mental Health Services
Program Name: Center Club
Schedule: 8:30-4:30 with one evening shift per week, occasional weekends and holidays.
Responsibilities: Assume primary responsibility for, and oversight of all Membership Services activities in the Membership Services and Education Unit, including facilitating work of the unit and managing both short- and long-term planning of Unit projects. Work with assigned members on goal planning including completing Action Plans, Service Notes and other documentation as required by the program and the Department of Mental Health. Review assigned membersí records as needed, minimally once per year. Enter data and review progress in accordance with outcome reporting requirements and quarterly audits. Assist in orienting new members to the program, as assigned by the Intake Coordinator. Maintain contact with membersí providers as needed. Provide vocational support and career counseling. Assist members in accessing community resources. Provide outreach services to Club members. Drive agency van.
Qualifications: Bachelorís degree, preferably in human services. Supervisory experience required. Ability to relate to individuals who possess a wide range of abilities and skills. Ability to work effectively and collaboratively with a culturally diverse population of members, other providers, and community partners. Conflict resolution or mediation skills. Excellent communication skills - both verbal & written. Flexibility is responding to needs of the program. Ability to satisfactorily complete a writing sample.
