Overview


THIS POSITION WILL BE HOUSED IN THE FARGO PATH OFFICE AND SUPERVISED BY NICOLE FEARING

The AASK Adoption Worker represents PATH on the AASK Collaborative upholding PATH’s mission and goals. The AASK Adoption Worker provides special needs adoption services according to AASK policies in a professional and ethical manner
Responsibilities

Resource Families
Recruit, access, train and prepare resource families to ensure quality care consistent with DHS rules and COA standards, PATH mission, values and quality standards, ethics and practice.

Participate in the development and implementation of local, regional and state plans for recruiting and retaining resource families.
Follow state and PATH guidelines for conducting family/homestudy, physical environment inspection and criminal background checks and provide required documentation for initial and ongoing licensing.
Coordinate required orientation and ongoing education and skill development for resource families.
Supervise resource families’ interaction with child(ren), child’s family, treatment team members and collateral contacts.
Monitor and evaluate resource families’ activities to ensure compliance with PATH and state standards. Submit critical incident reports and develop and implement corrective action plans as required.
Facilitate required team / unit meetings and participate in other resource families’ related activities.
Meet requirements for foster home visitation.
Provide crisis intervention as needed to maintain safety and provide for the best interest of the child and foster family

Case Management
Provide comprehensive coordination of all tasks required to meet the treatment needs and best interests of service recipients consistent with DHS, COA standards, PATH mission, values, policies, ethics and practice.

Comply with local office procedure for referral and service delivery.
Complete a systemic assessment of child’s and family needs.
Establish service parameters with legal custodian as child’s needs dictate.
Utilize the wraparound process in developing, implementing and monitoring the treatment plan for the child and family.
Provide complete and required documentation, within established time lines relating to case management and treatment planning.
Implement treatment services through the supervision of the child’s match / placement to meet the needs and goals of the children and families utilizing quality case management practice to provide for the best interest of the child and family.
Provide crisis intervention as needed to maintain safety.
Provide direction to other team members when applicable and make recommendations for discharge, finalization & permanency.
Meet requirements for youth / family contacts.

Caseload Practice

Ensure that paperwork is thorough and timely as required by standard within the timelines established.


Advocacy
Promote the needs of youth and families to ensure successful outcomes.

Ensure the youth and family have a voice in treatment and permanency planning processes.
Advocate legal rights for youth and families.
Advocate for resources to assist youth and families to successfully achieve treatment goals.
Educate the community about the needs of youth and families.
Advocate for the development and maintenance of funding and resources to meet youth and family needs at local, state and national levels.

Trauma-Informed Care
Conduct job duties from a framework that acknowledges the impact of childhood trauma on children, caregivers, and service providers. Integrates trauma informed care principles into practice by sharing knowledge and understanding of trauma with the child, foster family and relevant team members. Additionally, utilizes and advocates for the use of trauma-sensitive strategies to support the youth and their family.

Routinely screens for trauma exposure and related symptoms and demonstrates competence in using screening tool results to enhance service provision.
Routinely makes resources available and shares knowledge with foster families and providers on trauma exposure, its impact, and trauma-focused treatment.
Engages in efforts to strengthen the resilience and protective factors of children and families impacted by and vulnerable to trauma.
Emphasizes continuity of care and collaboration across child-service systems.
Understands the potential for secondary traumatic stress and practices self-care strategies to mitigate these effects.

Representation
Represent PATH’s mission and values with integrity in all interactions.

Develop and maintain community contacts to promote PATH and collaborative efforts.
Solicit feedback from clients and community partners to improve PATH practice & outcome.
Participate in CQI processes as assigned.

Supervision and Practice


Participate in individual and group supervision including clinical consultation to review cases and ensure best practices are provided to all children and families.
Provide timely and thorough information to supervisor regarding case incidence, service delivery struggles and difficult team processes.
Utilize supervision options and directives in service delivery.
Qualifications

Bachelors Degree in Human Service related field required.
 Current social work licensure required (LSW).
 Experience in foster care, adoptions and/or family-based service preferred (2 plus years
previous applicable experience). Consideration given for additional experience and
education (MSW or Masters in human services, psychology or related field).
 Demonstrated knowledge and ability to work effectively with children and families
having special needs.
 Understanding and ability to assess cultural differences as they apply to treatment
planning and service delivery.
 Ability to work independently and as a team member.
 Knowledge and skill in group facilitation.
 Ability to communicate effectively in verbal, written and electronic mode.
 Basic skill in PC windows environment, particularly in word-processing and e-mail.
 Ability and skill in documentation of behavioral health care and child welfare practice.
 Flexible schedule: ability to work nights and weekends based on the children and families
served.
 Valid driver's license, auto insurance and access to vehicle capable of safely transporting
clients.
