Social Work Case Mgr- Amily Group Decision Making/Adoption Opportunity

Job Details
Provides case management and direct services for children and adoptive families in a trusting and therapeutic manner, performing functions required by Federal and State laws and rules.

Acts on behalf of the Commissioner of Human Services to provide guardianship services.

Performs professional level case management establishing trusting, helpful, and empowering relationships with clients to assist individuals in success w/decision-making and self-determination.

Works in partnership with individuals or families to develop individualized case plans; coordinates and directs service to assure implementation of plans.

Serves as coordinator and facilitator of Family Group Decision Making Conferences for Carver, Scott, McLeod and Le Sueur Counties with the goal of developing realistic plans that address the safety, well- being, and permanency needs of the children. Arranges the meeting logistics; prepares meeting participants for the structure/format of the conference; clarifies the purpose of the meeting; and establishes bottom-lines with the referring worker and the family.

Ensures the safety of clients through the development of support plans, safety plans, and back-up plans; assists in times of crisis to influence positive outcomes.

Prepares and distributes detailed written reports outlining plans developed as the result of Conferences.

Collaborates with Child Protection Workers in the development of safety plans to address the safety needs of children in current child protection/child welfare cases as requested.

Maintains working knowledge of Federal and State laws and regulations and performs work accordingly.

Works the days and hours necessary to perform all assigned responsibilities and tasks.

Performs other related duties as required or assigned.


Job Description
Performs professional social work case management to children and adoptive families and in the coordination and facilitation of Family Group Decision-Making Conferences to address the safety and well-being of children.

Education Required
Requires equivalency of a bachelor's degree in a human service related field & 2 years of professional social work experience working directly with clients.

Experience Required
Strong preference for experience coordinating/facilitating family group decision making conferences.

Other Requirements
Master’s degree & bilingual abilities are highly desirable.

Must possess a valid driver’s license and provide a reliable means of transportation for the performance of work responsibilities.

Boating - Public boat landings are provided at Carl's (McMahon), Cedar, Fish, O'Dowd Lake, Spring Lake, and Prior Lake, as well as on the Minnesota River.

Regional Parks - Cleary Lake Regional Park features a nine-hole, Par 29 golf course; swimming; non-power boating; camping; trails; fishing; and picnic and playground areas. Murphy-Hanrehan Park Reserve features cross-country ski trails, horseback-riding trails, mountain-bike trails, snowmobile trails, pet trails, and turf-hiking trails. Spring Lake Regional Park is an undeveloped, 345-acre terrain with woodlands, marshes, and lakeshore.

Minnesota Valley Trail - when completed, will follow the Minnesota River from Ft. Snelling to the city of Le Sueur. The trail features picnicking, hiking, cross- country skiing, snowmobiling, individual and group camping, and horseback- riding trails.

Murphy's Landing - Murphy's Landing is an 87-acre, multi-national living museum depicting the life patterns of the people who lived in the Minnesota River Valley during the formative years of 1840-1890.

Snowmobiling - There are 105 miles of snowmobile trails in Scott County.

Bicycling – There are three trails in Scott County offering intermediate and advanced level riding - James Wilkie Unit, Murphy-Hanrehan Park Reserve, and Louisville Swamp.

Other Scott County Attractions include - Canterbury Downs – horseracing track, Mystic Lake Casino – casino gaming, Raceway Park – auto racing, Minnesota Renaissance Festival – festival offering a visit to medieval times, ValleyFair – amusement park.
